Dentist alleges Signature Smiles Humble, others misappropriated her TPI, NPI numbers for billing

HOUSTON – A Harris County dentist alleges her employers wrongfully misappropriated her identifier numbers to bill for services she did not provide.

Dr. Kathy Nguyen filed a complaint on Jan. 16 in the Harris County District Court against Terry Lee, Signature Smiles Humble PLLC, et al. alleging wrongful misappropriation and breach of contract.

According to the complaint, the plaintiff worked for the defendants at various locations. She alleges she has a unique National Provider Identifier and Texas Provider Identifier numbers associated with her name and professional identity for the purposes of billing Medicaid, Medicare and others.

The plaintiff alleges the defendants wrongfully misappropriated her TPI and NPI numbers in order to bill and receive payments for services she did not provide. She alleges Lee wrongfully profited from using her identity.

The plaintiff seeks monetary relief of more than $200,000 but less than $1 million and such other and further relief to which she may be justly entitled. She is represented by David Tang in West University Place and John Na and Pete Mai of Mai & Na PLLC in Houston.
